Rates start at just $7.50 a month.
If you choose an Anthem HealthKeepers HMO product, your copay for an inpatient hospital stay is reduced $100 for the first day of the stay. Also, you are eligible for an unlimited number of chiropractic visits for spinal manipulation and manual medical therapy.
If yours is an Anthem KeyCare or Anthem BlueCare plan, you receive important preventive care screenings (including checkups for members age 7 and older, gynecological exams, mammograms, colon cancer screenings, prostate exams and PSA tests) all covered with no co-payment required. In addition, if you have an accident that requires
outpatient emergency or urgent care services, Anthem will cover the first $750 of covered charges at 100%!
Retail Alliance® offers members tangible benefits. There are also some new programs of Life, Accidental Death, Short-Term and Long-Term Disability available at special rates for Retail Alliance® members with 2-50 eligible employees.
Upon enrolling in an Anthem program, Retail Alliance® members can log on (www.anthem.com) to access self-service tools, revise existing coverage, review bills and read the online newsletter. Employees can use the site to take advantage of interactive tools that will help them make the most informed decisions about their health care needs.

What The Benefits Include:
Eye Examination
A routine, complete eye examination, refraction and prescription for eyeglasses. Contact lens examinations require additional fees. The vision provider may recommend additional procedures, which are the responsibility of the member.
Eyeglass Lenses
Standard uncoated plastic lenses of any size or power.
Frames
Any frame up to a regular retail value of $100. Frames above $100 retail are available at an additional charge.
Contact Lenses
Any pair of contact lenses up to a regular retail price of $100, obtained from a network provider or the mail order program. Contacts above $100 are available at an additional charge.
LASIK – Non-Insured Discount Benefit
This non-insured benefit is offered at savings of 15% off the regular retail price or 5% off the promotional price when using the network.
Out-of-Network Benefit
While 97% of visions benefi ts are obtained from network providers, services received out-of-network are paid according to a fixed schedule (see chart below).
